TCC Family Health, founded in 1939, a Federally Qualified Health Center (FQHC), provides comprehensive, high quality health care and wellness services to those of all ages in the under resourced and underserved need in the greater Long Beach community. TCC is now a vibrant network of 14 clinic sites, including three school-based health centers, and one mobile medical clinic. Last year TCC served almost 40,000 patients through 175,000 medical, behavioral health, dental, optometry and wellness visits.

Position Summary:
The Program Manager reports to the Chief Health Education & Promotion Officer and is responsible for managing, implementing, assessing current and future programs that are: 1) aimed enhancing protective factors for families with children 0-5 years of age; 2) aligned with the mission and work of the organization; and 3) support the overall strategic goals of the organization. The Program Manager is responsible for Coordination and implementation of assigned quality improvement activities and ensuring that all aspects of the projects/programs are completed according to applicable terms and conditions. The Program Manager participates in team meetings, strategic program development and provides support with grant writing. This position is also responsible for overseeing the timely, cost effective and organized flow of health education material supplies and their cohesive display across all clinic sites. The Program Manager effectively collaborates with TCC leadership, medical providers, staff, and volunteers to provide patient experience training/development and foster a proactive approach to improve the patient experience, organizational culture, and community relations.
Leadership Role
- Oversight of health educators, program coordinators, community workers, home visitation workers, volunteers, interns and their activities and engagement of the community.
- Ensures compliance with TCC policies and procedures and regulatory agencies rules and regulations.Facilitates effective project coordination between internal departments.
- Promotes positive working relationships with community agencies and with other departments.
- Assists with emphasizing a trauma informed approach and environment that builds, supports, and promotes patient and family strengths and resilience.
- Participates in development design of programs for grant proposals.
- Participates in clinic quality improvement initiatives focusing on population health and clinical outcomes and data.
- Assists in preparation of departmental budget and cost controls in relation to clinical activities and supplies.
- Performs personnel management functions such as recommending personnel qualification requirements, drafting procedure manuals, and initiating in-service programs.
- Establishes and maintains courteous, cooperative relationships with staff, volunteers and the public, portraying the mission of TCC Family Health.
- Promotes delivery of quality patient care.
- Assists in improving UDS and HEDIS outcomes.
- Assists with other fundraising projects and assignments as needed.
Program Development and Management Role
- Manages Health Education and Outreach community workers and home visitation workers and their activities and engagement of program/project participants.
- Assists with management of existing and new program grants by: tracking grants (ongoing monitoring of grant program goals, hiring, monthly performance, budget and expenditures); developing project/program work plans; developing internal reporting systems; writing reports; maintaining excellent historical records; and working with staff to ensure each project or program is meeting proposal conditions and expectations.
- Assures timely and quality grant reports for relevant programs
- Oversees and/or leads health education classes and outreach activities
- Works collaboratively with Development and Finance Departments to manage program budgets.
- Tracks program outcomes as specified by grant objectives.
- Works collaboratively with other departments to ensure compliance with all program regulatory requirements, internally and externally.
- Works collaboratively with the quality improvement and compliance committees to measure, evaluate and improve program effectiveness.
- Must be able to communicate effectively within the health care community and social media environments as well as with patients and their families. This person must be willing to perform grassroots activities in order to meet the goals of the larger community.
- Represents TCC at designated events and meetings.
EDUCATION:
- Master's degree in Public Health, Social Work or related field or a Bachelor’s degree and equivalent experience
EXEPERIENCE:
- Minimum of 3-5 years related post graduate experience
- 3+ years’ experience in and understanding of effective primary prevention
- 3+ years of supervisory and project management experience, including, development of evaluation tools and evaluation of health programs, data analysis and report writing
- 3+ years’ experience in 0-5 home visitation or perinatal education and supportive services
QUALIFICATIONS:
- Ability to work collaboratively and communicate with clinic departments.
- Knowledge of community health needs, planning tools and approaches and methods, as well as public and community health models and frameworks
- Strong health care knowledge base required to assist with developing, designing and communicating health programs of all types
- Strong leadership and mentoring skills

- Must have passion for empowering under-served communities
- Familiarity with communities in Greater Long Beach and surrounding cities, including current challenges and opportunities

- Healthy Families America Home Visiting model training preferred
- Infant feeding, lactation educator or lactation consultant training preferred
- Bilingual: English/Spanish preferred
- Ability to travel

The physical demands of this position are those necessary to successfully perform the essential functions of this job. Duties requiring high tolerance and adequate coping skills for dealing with loss, grief, and bereavement. Sound stress reduction and management skills are essential. Also requires abstract thinking and intuitive sense.
Communication Demands: Frequent talking to clinical staff, patients, family, board members, funding agencies, community agencies and health professionals. Talking on telephone; giving group presentations, training/giving instructions; receiving instructions and writing composing written language (English and Spanish). Ability to work with diverse individuals and target groups such as mentally-ill, alcohol and substance abusers and homeless individuals.

Schedule:
Mon-Fri, 8:30AM - 5:00PM, some evening and weekends
8 hour shift
