**Title**
Acute Care Case Manager – Utilization Management
**Schedule**
Weekend requirement: at least 2 weekends in a 4-week schedule.
Holiday requirement: must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day).
**Assignment Details**
Census: up to 20–22 patients; ratio 1:20.
Travel experience: required.
Return-to-office/availability requirement: 7 days.
Pre-approved time off: one.
**Requirements**
- 2 years of acute care hospital inpatient Case Manager experience performing care coordination and admission reviews.
- Must have current acute hospital experience (worked in role within the last 6 months).
- Must hold a valid California nursing license (pending licenses accepted).
- Knowledge of InterQual and Epic.
- References: 2 references covering at least 1 year within the last 3 years (can include Charge RNs/Lead).
**Roles & Responsibilities**
- Perform care coordination and discharge planning for adult and geriatric patient populations.
- Conduct admission reviews utilizing InterQual criteria.
- Coordinate benefits eligibility and prior authorizations.
- Collaborate with interdisciplinary teams, including Social Services.
**Compliance / Notes**
- Scrub/dress code: business casual with lab coat OR dark solid scrubs with lab coat.
- LA fire card required within last 30 days (not reimbursed by facility/agency).
- Must disclose any license/background hits upfront; no open license issues will be approved.
